In MHA Episode 145, the plan, that was cooked to bring down All-For-One, and was put into motion, will see some transformation along the way and failure with it.
As, previously, in MHA Episode 144, Deku was separated from the designated team, which affected the overall plan.
According to the plan, Aoyama was to bring All-For-One with the help of Shinso’s brainwashing quirk, while Monoma would bring the heroes with the portal quirk copied from Kurogiri.
Heros will isolate villains using Troy Operation and Monoma’s portal, to bring them to different locations.
But a mishap happened with Deku(Midoriya Izuku) as told above.
bakugo and the team had to fight with Shigaraki Tomura in “Coffin In The Sky” without their trump card Deku.
But, they had to pay the price.

What Was The Plan And Why Did It Fell Apart?
Before we elaborate on the failure everyone went through in MHA Episode 145 because the plan went off the rails, we need to know the plan explained in MHA Episode 145 in detail.
We already know the gist of the plan, which is to separate all the villains with the help of Monoma using Kurogiri’s portal quirk, into groups and fight them.
What we don’t know, is the order, in which they will be separated.
The order was important because there was the question about some hero’s affinity with some villains.
As some examples, the following will suffice.
There were very few heroes, who could stand Dabi’s(Todoroki Toya) devastating flame.
Also, nearly nobody could comprehend Toga Himiko’s craziness.
So the details of the plan were like this.
What Was The Plan In MHA Episode 145?
Aoyama Yuga would trick All-For-One into coming out from his hideout, with the help of Shinso Hitoshi, while the absence of heroes nearby would assure All-For-One of safety.
Then, using Operation Troy as shown in MHA Episode 144, heroes will isolate all the villains momentarily.
Later, they will divide all the major villains into separate groups to deal with them, especially All-For-One and Shigaraki Tomura.
This step was necessary, since it was unclear, how much their consciousness was integrated at that point.
Shigaraki, being even stronger than All-For-One now, needed to be pitted against, Midoriya Izuku, Best Jeanist, Bakugo Katsuki, Edge Shot, and Mirko the bunny girl.
Additionally, Monoma will be using, Eraser Head’s quirk while copying it by staying in contact with him to render Shigaraki’s quirk useless.
On the other hand, they will take All-For-one to the remnant of the paranormal Liberation War, the Gunga Villa, and pit him against a team formed around Endeavor and Hawks.
Man of Devastating Flame Dabi will be sent to Kamino Ward to face Todoroki Shoto, his younger brother, and all sidekicks of Endeavor.
The crazy Toga Himiko will face an isolated battle on Okuto Island, with Uraraka and Asui with the leadership of Gang Orca.
The remaining villains will be sent to Dagoba National Stadium to face other heroes.
The villains, who were unable to be sent away, by heroes, will be attacked by the remaining hero team with the help of the Troy System.
As for, Kurogiri, held at Central Hospital, and Gigantomachia held at Jaku Hospital will be strictly under supervision by heroes because All-For-One can retake them at any time.

What Was The Reason For The Plan To Fail In MHA Episode 145?
The plan was all good, but it failed to some degree.
There are many reasons, why the plan failed.
The first and foremost reason was the unpredictable obsession of Toga Himiko, with Deku(Midoriya Izuku).
Her obsession(cough>love<cough), made her drag him with her into the same portal, to the same destination, which was quite far away from Deku’s designated location.
This caused the power balance of the team to dwindle, which was fighting against Shigaraki Tomura.
So Deku have to come back at any cost, without portal, because, Monoma was already using another quirk.
The second reason was, the increase of Shigaraki’s physical stat, even though his quirk was rendered useless by the joined effort of Eraser Head and Phantom Thief(Monomana Neito).
As for how he powered up, you would know if you have seen the episode, and if you haven’t, go and first watch it!!
Next was, the appearance of some unique Nomus with unique power, which was out of the calculation.

What Is The Failure Of Superhuman Society?
The battle may have only happened, between villains and heroes in MHA Episode 145, but the reason was far too deep and complicated to perceive.
MHA Episode 145 delves deep into the problem that had been, accumulated for a long time in the superhuman society.
Just like Akaguro Chizome(hero killer Stain) said, some people were doing as they please under the guise of law, pretending to be heroes or some of them were villains, with the lack of selective conviction.
Everyone was doing as they pleased, doing as they wished, going against each other, inclusive of hero and villain.
Someone wants to live freely, someone wants to change the world around, someone wants to just destroy.
All these voices of the proclamation, having no way out, being stagnant over time as rotten pus, gave rise to social unrest, thus, to the so-called “organized villains”, the failure of superhuman society.
Dabi was one of them.
